Are you struggling with obsessive-compulsive disorder (OCD) and have not made much progress in treatment?

Inference-Based Cognitive Behavioral Therapy (I-CBT) is a treatment approach that has been gaining a lot of recognition by demonstrating its effectiveness in reducing OCD obsessions and compulsions.

What Is Inference-Based CBT?

Inference-Based CBT is an evidence-based therapy that focuses on targeting the root of obsessive thinking or the obsessional doubt. Unlike traditional CBT or Exposure and Response Prevention (ERP), which focus heavily on exposure exercises and reducing compulsions, I-CBT works by challenging the faulty reasoning processes (or inferences) that give rise to obsessions in the first place.

This powerful approach teaches you about how normal doubts arise, how they differ from obsessional doubts, and how faulty reasoning maintains these doubts. During I-CBT sessions, you will learn how you use your senses for normal reasoning and be encouraged to trust your senses when it comes to obsessional doubts.

I-CBT differs from traditional treatment for OCD by:

  • Focusing on how the obsession starts, not just how it plays out anxiety and compulsions
  • Targeting “what if” thinking and irrational doubts at their core
  • Being especially helpful for Pure O OCD (obsessions without visible compulsions)
  • Emphasizing reasoning and reality-based thinking over doing exposures
  • Likely being more tolerable for people who struggle with ERP therapy

Who Can Benefit from Inference-Based CBT for OCD?

This modality is ideal for people experiencing:Scrabble letters spelling out "OCD" being held by two hands with an out of focus background.

  • Intrusive thoughts (unwanted, repetitive thoughts that create distress) about themes like harm, morality, relationships, identity, or contamination
  • Mental rituals, such as rumination, checking, or reassurance-seeking
  • OCD themes, including “Pure O,” relationship OCD (ROCD), sexual orientation OCD (SO-OCD), or existential OCD
  • Resistance or limited progress with ERP or traditional CBT

Whether you’re newly diagnosed or have been battling OCD for years, I-CBT provides a structured and individualized approach towards reducing OCD’s impact on your life, especially if you have tried traditional therapies.

Our I-CBT Treatment Process

Step 1: Individualized OCD Assessment
We begin with a comprehensive assessment to understand your obsessions, inference patterns, and compulsive responses.

Step 2: Identify the Faulty Inference
Together, we identify the initial moment of doubt or irrational reasoning that starts the obsessional sequence.

Step 3: Reality-Based Re-Evaluation
Instead of confronting fears through exposure, we help you challenge the obsession by reorienting to what you know is real, not what you fear might be true.

Step 4: Build Trust in Reasoning
We guide you in strengthening your connection to your senses, lived experience, and logic so you can better resist obsessional thinking.

Step 5: Long-Term Resilience
You’ll learn strategies to manage doubt, maintain insight, and reduce the risk of relapse over time.

Why Choose Our OCD Specialists?

Woman in green shirt, deep in thought, against a blue background. At The OCD & Anxiety Center, we are dedicated to providing specialized, evidence-based care for individuals living with OCD and anxiety disorders. Exposure and Response Prevention (ERP) remains the gold standard treatment for OCD and continues to be a cornerstone of the work we do. However, we also know that OCD is a complex and individualized condition. No single treatment approach works for every person, every time.

That’s why we’re excited to offer Inference-Based Cognitive Behavioral Therapy (I-CBT) as an additional treatment option in our practice. I-CBT allows us to better support clients who feel stuck in obsessive thinking patterns, particularly those dominated by mental rumination, over-analysis, or difficulty tolerating uncertainty. For these individuals, ERP can sometimes feel inaccessible or overwhelming on its own. I-CBT offers another path forward, either as a stand-alone intervention or in combination with ERP.

We view I-CBT not as a replacement for ERP, but as a complementary approach that can help unlock progress when traditional exposure-based work has plateaued. By helping clients identify the faulty inferences that drive obsessional thinking and reconnect with their senses and reality-based reasoning, I-CBT supports the development of insight, reduces mental avoidance, and strengthens the cognitive flexibility needed for long-term recovery.

Our decision to bring I-CBT into the practice came from a desire to continually grow in how we serve you, our client. As new research emerges and treatment approaches evolve, we are committed to integrating effective, evidence-based tools that meet the diverse needs of the people we work with. Members of our clinical team have pursued continuing education and consultation in I-CBT, and we’re actively incorporating it into care plans when appropriate.

When you work with us, you can expect:

  • Therapists who are trained in the most current I-CBT protocols and invested in ongoing professional development
  • Collaborative treatment planning that integrates I-CBT with other proven approaches like ERP and CBT
  • A warm, validating environment where you are seen as a whole person—not just a diagnosis
  • Options for in-person or virtual therapy across multiple states, with flexibility to meet you where you are

We’re proud to be one of the few practices in the region offering I-CBT. More importantly, we’re proud to offer it with the same level of thoughtfulness, rigor, and client-centered care that defines all of our work.

If you're looking for new ways to manage OCD—or deepen the progress you've already made—we’re here to help.
